Bola Health Loyalty Plan Terms and Conditions
Effective Date: October 19, 2024
The Bola Health Loyalty Plan (“the Program”) is designed to reward our valued members for their engagement and loyalty to Bola Health. By participating in the Program, you agree to comply with the following terms and conditions. Please read them carefully, as they govern your participation in the Program.
1. Eligibility
- The Program is open to individuals who are 18 years or older and are legal residents of Jamaica.
- By enrolling in the Program, you confirm that the information provided, including your name and email address, is accurate and up to date.
- Employees of Bola Health and their immediate family members may be eligible for the Program under specific conditions. Please contact us for more information.
2. Enrollment
- To enroll in the Program, you must sign up using the official Bola Health Loyalty Plan form, providing your name and email address.
- Upon successful enrollment, you will receive a confirmation email and will begin earning points for eligible activities as described in these terms.
- Only one account per individual is permitted. Bola Health reserves the right to close duplicate accounts.
3. Earning Points
- Points can be earned by completing specific activities, including but not limited to:
- Attending appointments or health check-ups at Bola Health
- Participating in wellness workshops and events
- Engaging with our Village Partners’ services
- Referring friends or family to Bola Health
- The points you earn, as well as the activities eligible for earning points, may be subject to change at any time. Bola Health will notify members of any changes.
- Points will be credited to your account within [X] days after completing an eligible activity.
4. Redeeming Points
- Points can be redeemed for rewards and offers as specified by Bola Health, which may include discounts on services, wellness packages, and other benefits.
- The current list of rewards and the points required for each can be found in your membership account or will be communicated via email.
- Points are non-transferable, have no cash value, and cannot be redeemed for cash unless explicitly stated.
- Points are valid for [12] months from the date they are earned. Unused points will expire after this period.
5. Membership Tiers
- The Program may include different tiers based on the level of engagement and points earned. Each tier offers specific benefits and rewards.
- Members can move between tiers based on their cumulative points during a calendar year.
- Bola Health reserves the right to modify tier benefits or adjust tier requirements at its discretion.
6. Program Changes and Termination
- Bola Health reserves the right to modify, suspend, or terminate the Program or any aspect of it at any time, with or without notice.
- If the Program is terminated, members will have [X] days to redeem any unused points. After this period, any remaining points will be forfeited.
7. Account Suspension or Termination
- Bola Health may, at its discretion, suspend or terminate your membership in the Program if:
- You violate these terms and conditions.
- There is evidence of fraudulent or abusive behavior in the use of your account.
- You engage in activities that undermine the integrity of the Program.
- In the event of termination for cause, you will forfeit any points or rewards earned.
8. Privacy
- Bola Health is committed to protecting your privacy. Any personal information collected as part of the Program will be used in accordance with our Privacy Policy, which complies with the Jamaica Data Protection Act.
- By participating in the Program, you consent to the collection and use of your personal information for the purposes of managing your membership, delivering rewards, and communicating updates about the Program.

9. Limitations of Liability
- Bola Health is not responsible for any technical or system issues that may prevent members from earning or redeeming points or accessing their accounts.
- Bola Health makes no warranties or representations, express or implied, regarding the availability or quality of rewards offered through the Program.
- In no event shall Bola Health be liable for any direct, indirect, incidental, or consequential damages arising from your participation in the Program.
10. Disputes
- Any disputes or disagreements regarding points earned or redeemed must be submitted in writing within [30] days of the transaction in question.
- Bola Health will review and resolve disputes at its sole discretion.
11. Governing Law
- These terms and conditions are governed by the laws of Jamaica. Any disputes arising under these terms will be subject to the exclusive jurisdiction of the courts of Jamaica.
